What Personal Information do I collect, and how is it collected?

To provide a safe and constructive holistic service for you, I need to collect and review Personal Information about you.

The Personal Information I collect includes:

Name, age, date of birth, address, and contact information (email, mobile/telephone number).
Relevant medical information.

I will also collect Personal Information from you when you:

Ask me for information about the services I offer.
Book an appointment.
Complete an online consultation form prior to your first session with me.

The Personal Information I collect may also come from you when you contact me via telephone, SMS text message, a contact form on my website, or through the Facebook Messenger facility on my official Facebook page.

To assist with the delivery of my services, it is necessary for me to employ third-party services. When you access these third-party services, they may also collect and process your Personal Information.

The third-party services I use are:

Setmore, to schedule and manage appointments.
Jotform, to build and create the online forms I use for holistic therapies screening.
Google Forms, to build and create the online form I use for pelvic health screening.
Square, the financial services platform I use to accept online and card payments, and to process invoices.

Why do I collect your Personal Information, and how is it used?

The Personal Information I collect is used to provide you with a safe and constructive holistic service that effectively meets your emotional and physical requirements, whilst not going against any medical conditions that are contraindicated for receiving massage or any other holistic therapies I offer.

I will also use your Personal Information to respond to your enquiries, discuss your consultation form submission with you, book, reschedule or cancel your appointments, provide invoices for the services you are booking, and provide receipts for the services you have received should you require them.

How do I store, share and disclose your Personal Information?

The Personal Information I hold is stored in one or more of the following locations:

Desktop version of Jotform.
Google Drive.
Setmore Booking App.
Hard copies of consultation forms are stored in a secured physical location.

Wellbeing Insurance (underwritten by Hiscox Insurance Company Limited) provide my public liability insurance, and state that it is a requirement for me to take and retain client records “for a period of at least five years from the date of treatment.”

After this time, electronic Personal Information will be destroyed by deleting:

All electronic consultation forms.
Setmore contacts.
All messages from Facebook Messenger.

Paper-based forms containing Personal Information will be shredded. Please contact me via the website if you have any questions relating to the retention of your Personal Information.

Sometimes, I may need to ask for professional advice on how best to proceed with certain medical conditions, and which treatments are most suitable for you. Before going ahead, I will inform you, and ask for your permission to share any relevant Personal Information anonymously.

If a medical emergency occurs whilst you’re receiving a treatment with me, it may be necessary to share relevant Personal Information with attending medical professionals to enable them to provide the most appropriate treatment for you. If a medical emergency becomes life-threatening, it may not be possible to obtain your permission before sharing your Personal Information.

Protecting your Personal Information

I will take all reasonable and appropriate steps to safeguard and maintain the confidentiality of your Personal Information, in its original state, and in alignment with your Individual Rights under the Data Protection Act of 2018. Your Personal Information will never be transferred or shared outside of the UK.

Individual Rights are your legal rights in accordance with the Data Protection Act of 2018. They allow you to understand the use of your Personal Information and can be exercised at any time.

Your Individual Rights include the following:

Your right to be informed about the Personal Information being processed, your right to object to this, and your right to restrict this.
Your right to access your Personal Information.
Your right to rectify, and/or, erase your Personal Information.
Your right to receive an electronic copy of your Personal Information.

If a substantial public or vital interest arises, this may precede your request to exercise your Individual Rights. Under such circumstances, I am legally required to allow access to your Personal Information for the purpose of heath related matters, legal matters, and law enforcement.
